Assessing the Needs of the Person You Care For

When deciding how much help or support you need as a Carer, Social Work will also take into account the needs of the person who you are caring for.

This Assessment is called a Community Care Assessment (sometimes a Single Shared Assessment) and can be carried out by either a Social Worker or Health professional. They will fill out a form that will ask about various aspects of the person’s condition or illness and how this affects their daily lives.

This Assessment can be carried out for anyone affected by an illness or disability, and whether or not a Carer is involved. However, the Local Authority is required to take account of your contribution and views before deciding on services to provide to a cared-for person. As a ‘key partner’ you have the right to be involved and consulted in issues affecting the person you care for, even if you choose not to have your needs separately assessed.

It is important to note that you are still entitled to have your needs assessed separately, even if the person you care for does not want a Community Care Assessment carried out for themselves or if they refuse help and support from Social Work.
