An emergency care plan ensures that you have a back up plan in place if you are suddenly unable to provide care. An emergency care plan ensures there is written documentation nominating who provides care, how to provide care and any other relevant information.
You will need the following information for your emergency care plan:
- Emergency contacts
- Health information about the person you support
- What care is required
- Any medications the person takes
- Regular support services
When choosing emergency contacts, ensure they are happy to take on this role. They will need to know what the role requires and be prepared. They will also need to understand any of your needs and wishes and those of the person you support, in case they have to act on your behalf.
On the emergency care plan you can record any expectations you have for the person who steps into the Carer role. Provide a copy of the completed emergency care plan to the emergency contact/s and keep the original in a safe but visible place - perhaps by the phone.
Remember, you will need to update the plan if any details change.
You can ask Social Work to discuss emergency planning with you as part of your Carers Assessment. Support Co-ordinators from Carers Link are also available to help develop and record an emergency plan - just e-mail or call 0800 975 2131
